it's best to use name as per IC. because in case of ID verification, nothing beats having the same name on the CC when you have to prove it's yours and an IC which is an irreputable proof that you are the name stated on yr CC>

Yes, it's better to put your IC name on the card for ease of verification.

don't think it matters, verification is usually done by the bank calling the point of sale and asking for details like your DOB, mother's maiden name etc
