By turning off all the safety systems on a RBMK positive void coefficient reactor with control rods that cause a power spike upon insertion. What made it worse is the reactor didn't have a primary containment structure. It's almost like they wanted it to happen. The operators didn't know enough about the design and let alone repercussions of their actions and went about testing its limits. The power spike they caused was over 33x rated core thermal power (core monitors maxed out, so that is a conservative estimate) -- super giant steam explosion from superheating the water instantaneously.

Truly, almost the most haunting and disturbing thing is the absolute disbelief and panicky denying of what actually happened. For all those educated men and women, its like sun wouldn't come out in the morning.
