Generally they are excluded. The card networks classify them as hotel charges, not restaurant charges.

Rare exceptions are possible when the restaurant is merely co-located with the hotel, is independently owned and managed, and maintains its own payment system. For example, if the hotel is located in or adjacent to a shopping mall, chances are pretty good that spending at the restaurants in the shopping mall will count as restaurant category.

There are a few hotels in certain countries that don't offer room service that have, instead, struck deals with nearby restaurants for food delivery. In some cases hotel guests can charge the food from these partner restaurants to their hotel room bills. That particular arrangement, if it exists, is quite useful if you're trying to maximize hotel category card spending rather than restaurant category.
