Have a look at your Cpuz memory tab, noticed that your NB frequency was at 4000MHz = your uncore frequency.
Your core was at 4.2GHz, so your uncore at 4000MHz, needs to lower down your uncore mutli by a notch. Differences must be 300-500Mhz for your uncore to the Core frequency.
You can look at my Cpuz above as example.

Get it run stable and understand your chip 1st, and note down your Vcore needed to run at what certain speed. Furthermore, run your memory at stock 1st, so not to confused with your other setting.
Vice versa too if you OC your memory, CPU run at stock.

ok. I am intending to do a reformat of OS as well. nVidia drivers are pretty screwed up as well.

I also wan to sort out the 125*34 issue. I will email gigabyte first. Cos this is really an error that shouldnt be happening. I will try the beta BIOS to see if its fixed.

Then i will start oc all from scratch again.

Or maybe you pass me your board and chip, I will try to configure and test it out. Pasir ris is where I stay.
Hope is not your chip that's giving you the bad, previously 3 guys whom I tested also needed very high Vcore, 1.31V for 4.5Ghz with 3570K.

yeah you need to know your chip, once you know it you can easily put it in other mobos as well and will easily figure it out

I am posting my own settings as example, u can used it as a guide.
Not all chips are the same, Vcore varies, speed and temp of your chip.
Mine is a delidded chip, so do note and is done on water-cooling system.

Maybe helpful to those who intend to OC or need it as a guide. Cheers

This chip I don't like and don't intend to keep it, I have changed at least less than 10 x 3770k ivy chips before....buy and sell away...part and parcel of my hobby...very hard to find a golden chip.

try disable all the C state and just enable Eist. Manual settings may help, Auto I don't like to used, tends to overvolt. LLC try turbo settings.

3dmk06 scored 35843! Msi R9 270Xtemp at a sweet and cool 55degC.
Do note Haswell at 4.5GHz run, the temp max out at only 64degC on core0.
Delidded chip works wonder on this Z87x-OC board
