clean your video drivers with driver fusion and reinstall latest whql video drivers.

Pls do a memtest to check any memory errors
You will need to run it for at least 1/2 hour.

Any over clocking will need to reset to the operating frequency.

both on the ram teeth and the slot..

I've found on the web that 'BCCode: 116' means Bug Check Code: 116 related to graphic card. Can i just roll-back to previous version?, mine's currently 332.21.

From the look of your system spec. Do some spring cleaning. Leave no dust inside the casing. Take out your graphic card and use a eraser to rubber at the gold finger until it is shiny again. Don't leave any eraser on the gold finger when you insert onto the PCI-E slot.

After a week of testing, i think i may have fix my problem(finger crossed). This is what i did.

Fixing the Nvlddmkm Error (Display driver stopped responding and has recovered) : Solution 5: Changing the time before it resets.
