You are asking a very important question.
And there is a possibility about the above scenario.
If the nurse involved did not check the past record before the injection, what other patients did she or he jab ?
Or could be a case of all of them are jabbed ?
We will never know the answer.

I guess it is a possible scenario.
I dun want to use "overworked" as an excuse as it concerned life and death.
However, it does indicate a lapse in the procedure since this is not the 1st time liao.
(I recalled similar case before this one.)
Did they rely on the due diligence of the worker to perform the check or common sense to expect them to check before jabbing ?
