Finally going back to Japan in the first week of November this year and having a headache planning where to go for 2 weeks. Does this itinerary look too packed? Should we stay in Osaka all the way and make day trips instead? Was thinking of replacing Kyoto with Hiroshima instead, but realised that the Miyakima Torii gate will be closed for renovation till next year :(

Also looking to go to Nikko as autumn usually arrives early in the first part of November there, but I've heard that Hakone is beautiful so I can't decide.

Day 1: Arrive Tokyo at 2pm > Osaka
Day 2: USJ
Day 3: Osaka
Day 4: Hiroshima Day Trip to visit Peace Museum
Day 5: Osaka > Kyoto
Day 6: Nara Day Trip
Day 7: Kyoto > Tokyo
Day 8: Tokyo
Day 9: Tokyo
Day 10: Disneysea
Day 11: Nikko or Hakone?
Day 12: Nikko or Hakone?
Day 13: Tokyo
Day 14: Tokyo > SG

I suggest u stay in osaka all the way to minimize changing accommodations. Both Nara and Kyoto are easily accessible from osaka.
I personally think Hiroshima deserves more than a day trip. Probably a night or two will be better. But since u mentioned the otorii gate is under reno then maybe visit Hiroshima again next time if tt is the focus?

I was thinking the same about Hiroshima too. That will leave us with a week in Osaka though. Would it make sense to split our stay between Kyoto and Osaka? Or would day trips be enough?

Do day trips from Osaka.

I would say Nikko over Hakone even thou I have not been to Nikko before.
