6 days in Kyoto seems a bit excessive. In 6 days you could cover Kyoto, Osaka and an additional detour or two to like Kobe or Nara.

Jr pass only cover Jr group rail. You can use hyperdia to check. If some place is far from Jr, you walk or can choose other subway. you seasoned traveler, travel so many day, by first week leg can get very tire, you may just want the closest station to your destination

Do 2-3 days in Kyoto, and then remainder based in Osaka. From Osaka can 1/2 day or full day trips to Kobe and/or Nara.

Day 16 - Kyoto Shopping
Day 17 - Tokyo -> Narita

I assume you are making the journey from Tokyo to Kyoto on Day 11?
So JR pass usage is from Day 11 to Day 17?

Yes, I would recommend you to get the 7day JR Pass, since you need to take the return trip on Shinkansen between Tokyo and Kyoto, plus it also includes Narita Express train to Narita Airport on your Day 17.

I also feel that you are spending too many days in Kyoto, when there's many other places to explore as well within Kansai region. There's Osaka, Nara, and also nearby Kobe. Since you have the JR Pass, you can also make a short day trip to Okayama or even Hiroshima if you want.

You are staying 2 nights at Kamakura? I only spent half a day at Kamakura and I'm already semi-bored. Also, bear in mind that accommodations at Hakone is pretty expensive (esp weekends) so staying 2 nights there might be a tad expensive.

Might I suggest something like this? To further utilize your JR Pass.
Day 1 (On arrival) - Yokohama, stay at Yokohama
Day 2 - Yokohama + Kawasaki, stay at Yokohama
Day 3 - Yokohama to Kamakura, stay at Kamakura (or just make it a daytrip to Kamakura, and stay at Yokohama)
Day 4 - Kamakura to Hakone, stay at Hakone
Day 5 - Hakone to Tokyo, stay at Tokyo
Day 6 - Tokyo, stay at Tokyo
Day 7 - Northern Tokyo, stay at Tokyo
Day 8 - Central Tokyo, stay at Tokyo
Day 9 - Central / Western Tokyo, stay at Tokyo
Day 10 - Tokyo, stay at Tokyo
Day 11 (JR Pass day 1) - Tokyo to Kyoto, stay at Kyoto
Day 12 - Kyoto, stay at Kyoto
Day 13 - Kyoto, stay at Kyoto
Day 14 - Kyoto to Osaka, stay at Osaka
Day 15 - Daytrip to Hiroshima+Miyajima, or Okayama+Kobe, or Wakayama, or Kanazawa, or Fukuoka, stay at Osaka
Day 16 - Morning Nara, then take afternoon Shinkansen back to Tokyo, stay at Tokyo
Day 17 (JR Pass day 7) - Tokyo -> Narita
