If you meant to buy applecare+ upon buying your phone, it can be done at the counter on the spot. But once you settle payment without it you cannot buy it from anywhere (apparently, and I don't know the logic behind it), and you'll have to call Apple to buy it; within 60 days of course.

Edit: This is from my personal experience. I didn't get the applecare+ at the Singtel event, and decided to get it only somewhere earlier this month. Tried buying at Nubox but can't buy it due to some policy. Person told me to call Apple to register for Applecare+

Yes, the carriers are able to process the AppleCare+ for you but only if you purchase it together when getting the phone.

If not during the first 60 days if you wish to purchase the AppleCare+, you will need to call Apple and they will run a diagnostic on the device before activating the AppleCare+ and charge your credit/debit card.

Resellers will not be able to activate AppleCare+, due to not able to run the diagnostic test on your phone. Requires Apple to initiate the test.

If purchasing AppleCare for MacBook/iMac/Mac Pro can buy from reseller. Just provide them with the serial and they will process it after payment.

Provide them with serial number or imei. Since all iPhones are activated thru their server they can push a request just like an app notification. Kinda like Apple 2FA, where they will push a request to your device.
