Has anyone redeemed DBS points for shopping vouchers?
I have redeemed mine and received a form in the mailbox where I have to fill in personal information instead of the typical gift vouchers, will this form be used as a voucher?

The last time I redeemed DBS vouchers. DBS just sent a form telling me where to redeem it. If I am not wrong, the place to redeem your vouchers is at Funan.
