I purchased them at convenience store most of the time and the store keeper will warm at my request. Never purchased at dept store and train station.

For dept store and train station, I do not think they can do that.

If you are purchasing the bento at the convenience store, the cashier will usually offer to warm the food for you.
ekibens, on the other hand, are usually sold+eaten cold. eg Bentoya Matsuri at Tokyo Station does not warm food/have a microwave to warm food.
And yes the food is precooked and ready to eat (when cold) before the due date. Kinda common sense I would think.

MOst of those bought in shops inside the train stations (before and after entering the gantry), are not warm nor hot. Some are chilled. But so far, they still taste great.

I believe this is also because if everyone takes out hot bentos and start eating, the whole train will stink with a mixture of aromas, which not many people like.

self-heating bentos are one of the coolest things i got in japan. it got too hot to put on my lap and condensation was dripping all over. i remember it was a unagi bento =:p=:p
