Looks like you have tried a lot to have this fixed but no success. I can't really see any similar cases so this seems like an isolated issue. And the symptoms points to me a software issue, small chance this is a hardware issue.
If you have not done so, you can try this:
1. Do a time machine backup of your system
2. Perform an erase and install of OS X
3. DO NOT restore from time machine, setup as new Mac
4. Keep running software updates until it's up to date
5. Attempt the bootcamp installation again.
Important: Make sure you do the erase and install properly and do not restore your data, make sure you got the correct 64 bit Windows ISO installer file.
If the above steps still fails, I would bring this up to Apple Support. Apple engineers should attempt to capture your Mac and try to see what's going on. Basically you've done all you can to isolate the software/hardware issue.
