Charge while battery being drain

Why is my phone starts to act weird? Charge while draining? I charge my phone it go down quickly by 1% not rising. Any solutions? My phone is note 2. It take so long 1 amd a half not even 50% and up. Stucks at 15%.

maybe batt prob... or charger fault... try change charger and see how, if not, then change batt... IF BOTH still can't... might be charging port or any other hard ware issue.. imo
