Completed a task, hit KPI but mission failed

Can management experts advise how to avoid such scenarios?

For example
Task = Design bento box for school chewren
KPI = Meet healthy guidelines

Outcome = Bento delivered but costs more and condemned by customer.

Another example
Task = Decapitation strike
KPI = Kill as many enemy leaders as possible

Outcome = Leaders killed but enemy still fighting and give you CoL+1000 dmg

Oh KPI works like this:

Task = Design bento box for school children
*Matrixes:
1) Must have 2 vege, 1 meat, 1 rice, 1 soup = Weightage 20%
2) Cleanliness and hygiene to observe = Weightage 20%
3) Food selection need to meet health requirements = Weightage 20%
4) Customer Feedback = Weightage 20%
5) Costs = Weightage 20%
* Key matrixes gather from survey from stakeholders including customer aka students

Task = Decapitation strike
**Matrixes:
1) Must eliminate every level of leadership that will threaten regional peace = Weightage 20%
2) Must remove nuclear threat = Weightage 20%
3) Must cut off Iran's proxy = Weightage 20%
4) Must not harm civilian = Weightage 15%
5) Must not affect global oil prices = Weightage 10%
6) Must not result in long term war = Weightage 5%
7) Extra matrixes = Weightage 10%
** Key matrixes gather from Ministry of War and Israel

Note different dept will have different key matrixes.
For example for Israel might be:

1) Must eliminate every level of leadership that will threaten regional peace = Weightage 40%
2) Must remove nuclear threat = Weightage 40%
3) Must cut off Iran's proxy = Weightage 10%
4) Must not harm civilian = Weightage 5%
5) Must not affect global oil prices = Weightage 0%
6) Must not result in long term war = Weightage 0%
7) Extra matrixes = Weightage 5%

That why Trump and Bibi need to negotiate the matrixes for each other's benefit.

Whether task pass or fail depends on stakeholders, for the canteen case, its flawed as there is no matrix given for customer. For war case, the outcome has yet to conclude to make judgement. TS may judge based on his own matrix but that doesnt mean it is what the business / war is intended for.

fullfilling KPI is just like checkboxes, do the minimal that fulfill its requirement. Last time my IT dept KPI is to submit incident report within 3 working days. They fullfil the KPI of 3 days, but the report is sloppy

as long as the money is already in so....... 🤷‍♂️
