Just get the form and fill it in, sign and send back to Aviva.

I don't need reward, I just need it to be able to be charged.

If I’m not wrong, based on my conversation with Aviva, credit card payment doesn’t apply for MINDEF/MHA plans. So, I’ve been paying via GIRO/bank deduction for years...

Not yet. Scb exclude mcc 6540 wef 2 may
