[Help] My GPU is confirm dead?

When turning on, monitor super glitched up until it hits BSOD with this error just before reaching desktop

Manage to go inside safe mode, this is what it looks like

Right now my monitor is fine again.. after reinstalling driver software. Expecting it to spoil anytime again soon as it's been occurring for past few days.

Wanna narrow down if it's really GPU issue before deciding on next action.

gtx 580, corsair 650w. whole system was built abt 4 years back.

Yea, it's your GPU.
Just happened to me too
I tried using the internal GPU, my mobo has 1, and all is ok.

Switch GPU to use the other slot, still same issue.
Next is, buy new GPU

One of my GTX 580s also started to malfunction, giving the same kind of horizontal lines. The brand is Gainward and its 3 years old now, so I don't think got any warranty left.

My screen also like tat last week , end up i find out is the bloody Hdmi cable spoil
