Help needed to generate pdf from user input values in website

Hi

I am currently creating a website for my company.

I like to have this function that allows user to key in certain information eg. names, contacts, etc and they will click "generate", then a pdf document would be generated from those information they keyed.
The document generated would be based on a standard template document that is same for all customers. Only certain information will vary based on what the user key in.

Is there any services online that may have such function? Please help. Thanks.

What technologies are you using to host your web app?

You may want to use the following approach.
1) Make sure of mailmerge, fill up the fields in a word document
2) convert the word document into a pdf

for some projects i used wkhtmltopdf bin installed on linux

U can try to generate the PDF using Javascript, based on your description this should suffice ~
