How to create custom ringtone in iphone 5?

Hi, i use to be able to choose my song as ringtone, but after reformat my pc , with the new itune install, i am not able to do it anymore.

I did not jail break my phone, so how do i set e.g. my fav song a ringtone/alarm tone? Any apps to download?
