Say you have a scheduled day surgical operation during one of high key ICT days, which you still can work immediately after the operation. All you can do is to speak to the CO and ask him for time off right ? Because I seriously have no wish to defer, just want to complete the surgical operation asap. But worried they might not let me go if the schedule clashes.

How did you end up scheduling an operation, presumably elective since you can wait for so long, earlier than the 6 months notice period?

As the other users have mentioned, your surgeon will most likely issue u an MC to cover his backside.

As for whether your CO will discharge u from the ICT, it may not be the case. Do speak up with him and explain your circumstance. He still has the right to decide and may decide in your favour.

for your case, usually the CO will ask you to report to MO to endorse MC and at the same time, MO will decide how many days of MC you need.

this will form the basis of his consideration whether to out pro you or not.
