Need Help Regarding Graphic card problem

I need help regarding confirmation whether my graphic card has died :/
System specs as on the right.

For both the pci e slots on motherboard, the system also shows the same problems. If it is the graphic card problem, what card should i change to?

Hmmm then any suggestions on what brands to get?
Thinking of a Gtx 760ti or a gtx660ti, depending on which is more value for money.
my little 520W should be able to power it too i guess.

Gtx 760 performance shld be almost on par with gtx 670 or slightly better. If u can afford, gtx 760.
