NEED HELP! Screen Freeze

Recently while watching shows or play full screen games, my screen would either freeze while the sound continues in the background, or the screen would freeze with a loud buzzing noise without recovery. Sometimes, the screen would just go into power safe mode.

My parts are as below:
CPU: Asrock H81M-HDS + i5 4460
RAM: G.Skill PC 12800/1600MHZ DDR3 CL11 Value Ram (8GB)
GPU: Sapphire R9 280 3GB Dual-X
SSD: Plextor M6S 128GB
HDD: Toshiba 1TB 7200Rpm
DVD-RW: LG 24X Dvd-Rw Sata OEM
PSU: Superflower Hx 600M Gold Modular
OS: Windows 7 Home Premium

Open your case see a sapphire r9 280 and make sure u remove all the plugs and screw that is connected to the GPU and then make sure ur GPU slots does not have that lock (white color piece of stuff to lock at the back of the gpu, if there is try to pull that white piece back b4 u remove your GPU.
