I need some advice on whether we can see Okinawa without renting a car as my friend and I are not confident in driving overseas.

Also, is 5 days enough for Okinawa?

Not impossible, but will be very difficult to go beyond Naha.

I've taken public transport in Okinawa (from Naha to Chatan area) and I would not recommend it unless you are willing to do extensive research on bus schedules or know Japanese.

An alternative is to join those one day bus tours that depart from Naha daily.

The best way to explore Okinawa would still be to drive, and the roads beyond Naha are often not crowded and quite easy to drive.

U can join bus tour

Now there is direct bus from airport to the famous aquarium but limited timing n $$$
Car daily rental still cheaper

Can hire driver or take taxi but limited availability.

Also, need IDL for renting car in Japan.

If its jus 2 pax and no elderly or infant -bus tour is fleasible. As per stated, limited timing plus very time consuming and not so cheap. (2 pax is still ok). No worth the cost and time of in group of 4-5.

Can plan to stay in Naha 2-3days and go stay northen in Nago 2-3 days and schedule bus back to naha airport. Can be done but extensive reseach and planning needed.
