How did it pass the SOP when it 1st established?

As usual, the news indicates that there is either no SOP or it hasn't been followed closely to ensure safety standards. Hopefully, the same thing won’t happen again.

Is toxic and the maintenance team did not do risk assessment prior to going down the chamber

i guess they do not have SOP and RA for maintenance, whereas for normal operations there is something in place

The pigs are lowered by a machine, called a carbon dioxide stunner, into an underground chamber that gets filled with carbon dioxide to knock them unconscious. This prevents the pigs from experiencing pain or distress during slaughter.

On July 19, 2023, a maintenance team was carrying out troubleshooting works on the stunner, which had broken down, when a metal bracket fell into the chamber. This occurred while some bearings behind a sliding door were being replaced.

At around 2pm, a maintenance worker, Mr Kathivaran Narajan, entered the chamber via a manhole, and climbed down the vertical ladder to try retrieve the bracket.

He experienced distress after around 30 seconds and was pulled out of the manhole.

The carbon dioxide is supposed to prevent pigs from experiencing distress. And yet a human experienced distress inside. So how does it work for pigs and not for humans?

This company is owned by government so there is no worry about the authorities as long as nothing goes wrong. It operates with no government and don’t bother to comply with law.

The Malaysian guy who died, was the one who got lazy and tried to take a shortcut.

Seen too many such cases IRL, but they always get away with it as the scenario is not life-threatening.

Primary Industries was set up 54 years ago by the government to handle single abbatoir operations in Singapore. Now, it is owned by Singapore Food Industries and SATS.

Good to see Singapore can go after our own if they are not compliant with the law.

cheaper than firing a worker :)

Actually this reminds me of a case from > 20 years ago, came out in the newspapers … a surveyor onboard an Indian crew chemical tanker loading cargo at 1 of our terminals, ship’s tanks loaded 1 foot of cargo for 1 foot sampling already … surveyor never strap on his helmet and when he lean in front to take the sample, his helmet dropped into the ship tank - the ship crew following him (to open the tank hatches) went into the tank to help him get his helmet back, end up suffocating from the cargo fumes and died inside.

I know this surveyor personally :s14:

$15k fine for an accident that causes loss of life. I just find something is very wrong...every time when there's an accident and someone gets hurt, they make the responsible party pay money to the gov, the victim gets nothing... :rolleyes:
