Built this PC on Dec 2014 spec below
ASROCK H81M-HDS
i5 4460
kingston 4GB x 2 1600mhz ram
wd 1tb blue
samsung 850 evo 500gb
msi gtx 970 with 362.00 driver
seasonic m12ii 520w
S2340L Dell monitor
window 10

Now I am having some weird issue for past few days. Getting display driver crashed and recovered when idling at desktop. At first I thought it was display driver so I DDU and reinstalled the driver same issue still happened. Next i tried to play Witcher 3 and Xcom 2 for 30mins no issue 60fps most of the time. I checked Bios on 12V rail and it was showing 12+v.

What can be the issue here ? Graphic card or PSU dying ? I do not have spare with me so I cannot replace parts to troubleshoot.

Thanks for helping

have u tried to remove gfx and try onboard gfx? suspect might likely gpu though...seen a few with this case, if not try to clear the contact of ur rams and gfx and test. otherwise try using 1 ram at a time.
