tldr: Something to upgrade my rig so i can play current AAA titles. Play game + watch youtube on 2 screen concurrently without major hicup, can compromise on game details.

Hi Peeps. Been a long time since i've been here.
Currently i'm still running the rig i've made back in 2010 (or was it 2009?) as per the right side, with a 1920*1080 + 1360*768 Monitors (At native resolution).
Currently this rig has been struggling to even play 1080p videos on youtube(html5) without stuttering on many browsers such as chrome or firefox, making me jump back to Opera, which alleviates the problem for now.
Also, i can't play many recent AAA titles(currently fallout 4) without lag or stutter without closing my browser, disconnect one of the monitor and downscale my main monitor (1080p one) to a lower resolution, and play it windowed at a resolution even lower, at the lowest settings plus tweaking the settings from windows. heh
Even doing all this, the rig fails to run smoothly at many times, especially when i'm in the open world.
While the game was running i opened up task manager, and saw that it seemed like the RAM(Yes definitely is) and HDD was running almost at full capacity/capability, so i'm quite curious:
[Question 1]: Is my HDD limiting the performance at this point of time also?
[Question 2]: My GPU probably doesn't have enough memory to support such high resolution games, and even monitors for heavy game + browser usage currently, safe to say?
[Question 3]: I'm not really intending to overhaul the system, so what incremental upgrades can i do to fulfill my needs of (1) Multi screen display, (2) Youtube + AAA Gaming (no need for maxed settings, but preferably run at native).
[Question 4]: My father stole my heatsink so i'm using the stock AMD jet loud AF blower, any cheap recommendations? $20-30 is fine.
Currently i saw the Zotac SSD, palit 750ti recommendation from lazada from the other thread, will those be a significant upgrade from the current rig, and if so, how much?
Thanks in advance
