i have tried it during mobile replacement on the day itself, CS took care of the activation.
not sure if that's the case now.

No what I mean is after signing up, does anyone tried the replacement service before? What’s the template of the service request form like?

template ? just call smart support
say you lost or need a replacement phone, give them your cc info and that's it.
phone will arrive in 1-4 hrs depending on location.

Hmm that’s weird. I emailed them and they told me to filled up some confirmation of service form without header. So I’m not sure how legit it is.
