Not much difference in December than any other period if you're only planning Tokyo. Some more you never mention how long your holiday is, or whether it's early mid or late December.

what are your interests?

autumn leaves?
shrines temples?
shopping?
anime/manga?
food? what type?

if you are staying at disneyland resort and have the money, please consider the Mira Costa. you will not regret

Disney itself is gonna take up 2 days..Assuming you have a full 7 days in Tokyo..Pretty easy to fill.

But bring more lotion and lip balm for the kids..

Outlet shopping i will take Gotemba Premium Outlet..

Stay in shinjuku or ginza with good stroller access and baby facilities. Can consider Disney affiliated hotels with free bus transfers instead so no need to change hotels.

MiraCosta? Now? I don’t think you can get a room with good view unless not fussy with just a family room facing the carpark. Or unless you managed to book Vacation Package.
