Are you referring to the card collection option or membership no.?

The card collection I saw it previously when i tried to redeem but failed when it was still the $200 dining vouchers. Now the form doesn't ask. If I really did miss it the form shouldn't be allowed to be submitted in the first place since it is missing information.

The eligible restaurants are listed in the TnC for the welcome reward
