Sometimes it is fated. If you refer to the timeline, she had already caught flu before going to Japan (mentioned as 'cough'). But because they are rich, she was admitted to a hospital when they reached Japan. So I believe there was no delay in the treatment.
Rich or poor when time is up Yan wang says so who are to say otherwise. No doctors or lawyers can nego.

how does flu become pneumonia ???
Pneumonia is a lung infection

Influenza is a common cause of pneumonia, especially among younger children, the elderly, pregnant women, or those with certain chronic health conditions or who live in a nursing home. Most cases of flu never lead to pneumonia, but those that do tend to be more severe and deadly.

Many times, the cause of death is pneumonia even one do not have flu and influenza. That is due to failing lung functions too.
Barbie Tsu hasn't even very healthy. That is probably one big factor where she fell ill and the condition deteriorate quickly.

Actually it's possible for even normal ppl to get pneumonia during flu and need to hospitalized
Yes, I caught it once when I was 15 years old. I was playing soccer in the rain and that night I jumped out from my bed like being possessed as I cannot control my hands and legs. Luckily I still can shout for help. Warded to hospital and for the next 3 night around 1am I will suddenly wake up and my temperature shoot to 42 deg. ++.
