Think with dementia, the process is more troublesome. Not that straightforward, as they deem that the person is not able to make that decision on own.
yar , LPA need doc certified that the person is able to make the decision to appoint the LPA .

Yes it happens my grandpa ever did that.
he forgot where he is after going out at 1pm.
Sat at the mrt station to figure out where he stays.
He remembers at 9pm and took the mrt home.
MRT station and Bus interchange is a Dementia Go-To Points.

So it would be good to train persons living with dementia to approach the staff for assistance when getting lost.

Staff undergo training to recognise and support individuals who suffer from dementia. They are equipped to ensure the well-being and safety of these individuals, while making their best efforts to reunite them with their caregivers and loved ones.

My dad in his 80s had been missing since 18 June (Wed) morning, met his peers at a coffee shop in Waterloo Street near Kwan Im Thong temple. He lost his way while trying to make his way home, and last seen riding a bus near Loyang Avenue followed by Jurong. He has dementia and other medical condition requiring daily medication,
He is wearing short sleeve in blue colour and trousers in dark colour. Police is now tracing his whereabouts, anyone who spotted him, please bring him to the nearest police station .
Your help will be truly appreciated, please kindly spread the word. Thank you
🙏
Some pictures so we can help to look out
