Multiple gaps were found in how various agencies in Singapore had handled the case of four-year-old Megan Khung, who died in February 2020 from child abuse.

This is the conclusion by a review panel, tasked by the Social and Family Development Ministry to look into their actions, protocols and processes. Following the review, two police officers were disciplined for not following protocols.

A child protection officer from the ministry is also going through a disciplinary investigation. Ms Pang Kee Tai, Deputy CEO at PAVE Integrated Services, talks about the biggest lessons that child protection practitioners should take away from this tragedy.

The pre school when saw injuries did call up Child Protection Services when required
When blood relative like grandmother suspected something, she did call the police

understandable reply. this case involves complicated emotions and i don't want to assume anything. it's intended to give simonboy some peace. it's all over and i'm sure he feels pain reading this news over and over again. worse part? can't bring back time. he can only live with regrets. it will haunt him forever

it's not that people are scared to call police, but the police usually show up only when there's blood splattered or somebody died already. if you haven't die, it's probably just an additional case report that cannot be closed. singaporeans love to complain. spf is overloaded i think

it's very sad as sinkie so kiasi. they kept pushing around who to call police, grandma don't want then all give up..
for me even if I'm a stranger on roadside seeing 4yrs old with so many obvious bruises not like fall from bicycle/running , I will even call the police.
Her teacher will suffer from guilt for the rest of her life..
