This post on google product forums explains what constitutes to a rollback loop after OTA in my case where boot slot b had write issues and how to identify it. In case anyone faces the same problem in the future.

The context: with a locked fully stock bootloader, samsung ram/flash.
With my previous clean reinstall of feb factory image + march OTA today, no issues with slot b so far!

Looks like I did corrected the bad OTA. So its not a hardware issue overall.

What was the issue with Feb update?

I just updated. My build number is NOF27B now. Is this the same as yours?

Yup. Summary was somehow I ended up in a rollback-OTA (slot a-b) loop because the second of the dual boot slots with the pixel had write errors likely from a bad OTA. I had to do a full reflash of factory image to fix that without any idea if it will reoccur in march OTA but thankfully it did not.

Also reported and trending issue on google forums, not sure if this can still possibly happen with NOF27B but at least it allays my concerns of it being hardware and is actually correctable with a reflash fortunately though its regrettable this happened.

Ever since the 5X bootloop and this issue, I think I am more skeptical of getting the latest update. In future, I will just delay my update by a few weeks to monitor the latest update.
