Hi,renfred89, I was only diagnosed with this condition just a few days ago, after I suffered breathlessness when doing my morning swim. Usually I could do 20 laps with no problem, but that day I was out of breathe after only 3 laps. To cut a long story short, I was admitted to hospital after my blood test showed a raised level of a chemical called troponin (I think that's the spelling) and ECG reading showed a possible heart attack. Then after an angiogram done to show that there was no blockage of tne main arteries, they sent me to do an cardio echogram, then they realized that I got thickening of the heart muscles. The specialist advise me to get my kids to do the same tests to confirm that they are ok. For my girl who is in sec4, he gave her a memo for the polyclinic so that they can refer her to the SGH Heart Centre. And my boy,a memo to the MO of his camp.
I hope this clears it up. If you are correct about my son's ecg that was taken before his enlistment shows all clear, than I am all for it. But I think my cardiologist still have a point there. I think I will call his camp hotline on Monday morning to see what should be done. Thanks everyone.

I know your main concern is to ensure that your son is not affected. However your son has the right not to pursue any form of medical investigation even if you insist. Your call to the camp will only jeopardise his plans for training and incite unnecessary fear in the unit.

Instead of calling the camp, why don't you understand more from your son first and talk to him nicely?

HCM doesn't occur overnight, it takes years to present itself with the first symptom.

Let's assume bothy you and your son agree for further check up, you need not alarm the camp or the in-house MO yet. You could bring your son down to any poly clinic for a referral to a cardiology specialist outpatient clinic for further examination. In the mean time, an ECG can also be done at the poly clinic to rule out any immediate anomaly. I don't think poly clinic draws cardiac enzymes (troponins), but you gonna be sure by checking with them.

Hope my input helps.. :)

Ey, dunno anything dun anyhow put

1. in Singapore, the age of consent to refuse medical assistance is possible only >21 years of age. So the TS probably decide for his kid unless his kid is >21 years of age.

And what kind of fear in the unit are you talking about? It is not as if TS son's got a contagious disease.

2. HCM doesn't develop overnight, but with at least 18 years of his life gone, it might have develop to an extent pre-disposing to atrial fibrillation which is the real killer. All you need is just one aberrant cardiac fiber...

3. ECG is not suitable in this case for diagnosis purpose. Neither is troponin ( actually, they use CK-MB) and they are release AFTER some heart muscle died. It is not diagnostic of HCM. Dunno, dun anyhow say. And as long as the polyclinic can draw blood, they can check for this chemicals in the blood by a pathology lab ( infact, even ur GP can do it)

4. You should call his unit directly and inform the OC and explain the situation to him. Trust me, the last thing the OC wants is a dead boy on his watch, esp if the parent has called up and explain the situation; not very good for his career.

Hi again. I thank you for your reply. As I had said before in my earlier posts, it is the heart specialist that attended to me in the Heart Centre that had suggested that both my kids get screened as soon as possible. In the young, this A_HCM is the major cause of sudden cardiac death. (According to the specialist). No doubt it don't just occur overnight, but it's better to be safe than sorry. Cheers.

Hi, cosycatcus, you a doctor? I think your explaination is the best of all. Yes, I will talk to his Unit on Monday. I really don't mind him persuing his dreams of getting sgt's stripes, but I want him to be safe too. That is every parents' hopes. Cheers.

1. The law is only valid when a minor has a confirmed diagnosis. TS's son has the right to refuse being tested.

2. Atrial fibrillation has no connection with HCM. Moreover, AF does not manifest paroxysmally.

3. Are you sure any GP has the resources for blood test instantly? Are you sure CK-MB is usually used for HCM diagnosis? Do you even know what CK-MB stands for? Lastly, do you know when CK-MB peaks and how long it returns to baseline? Do you know which is the most sensitive test? These are not called chemicals, these are cardiac enzymes.

ECG is not the sole diagnostic tool, but it indicates abnormal morphology which warrants specific investigation. Do you know how to tell if an infarction deviates to which axis? Do you know the normal duration for a QRS complex? How about a PR interval?

No doubt the parent can inform the unit, but with no medical evidence on hand, the unit will send the serviceman down to the MO for proper documentation. I am sure another ECG will be done to evaluate his cardiac condition.

If you're a healthcare worker, or a student pursuing in the healthcare industry, you just got slapped by me up down left right and centre.. You have the knowledge, but you used them wrongly. Beneficence and non-maleficence are 2 bioethical principles you've violated.

So, this morning, after persuaded, my son went to the MO. Letter (memo) delivered. It's now left to see what the MO will do.
