The theme is called "Elite Pro HD", a paid theme US2.99. It's for iPhone 4, not sure whether does it work on other version.

As for the BiteSMS icon, I didn't create it myself, I found it somewhere, downloaded it and changed the icon.

I dont blame the dude. 3 years ago, with the iPhone 2G, the only theme I ever used was the Reflective dock. The rest was completely stock. It's really clean and nice on the eyes.
