Yes calendar month.Their exact statement as follows :
"Each account has only 1 billing cycle, starting from SIM activation date.
For example, if a line is activated on 19th Sep, charges from 19-30 Sep will be pro-rated in the first bill.
There is only 1 bill cycle which is Cycle 1"
My interpretation is that they now follows calendar month, but the way they describe it is certainly open to interpretation.
First list makes more sense to me. 2nd list for Roamlite. Not sure what is that.Zero1 has 2 list of preferred roaming partners.

Took about 10min plus, all done and activated. $5.45 fee.
