Think from the shaky video looks abit like the cyclist cycling vigorously never really look in front, when van finally cut in front, the cyclist panic, brake and swerved to the right trying to avoid, but caught off balance and buang.

the van driver is turning left, so the cyclist has to filter to his right but just as the van turning action cause it back side to hit the bicycle or the cyclist filter too early and hit the back of the van
